(You will need a full name and address for the alleged victim. CDSS does not investigate reports of abuse, so enquiries or reports should be directed to the appropriate county APS program.

To Report Institutional Abuse, Call:
Call: The Long-Term Care Ombudsman (LTCO) program for the county in which the alleged victim lives. Information about the LTCO (including telephone numbers) is available at: http://www.aging.ca.gov/html/programs/ombudsman.html.

Reports may also be made to (916) 323-6681 between 8a.m. and 5p.m. Pacific Time. Please be sure to have the alleged victim's name, the name of the town, and the name of the facility in which the abuse is occurring.
